Pre-Participation Physical Evaluation

Make sure athletes are healthy enough to play

The Texas UIL and other sports leagues require student athletes to undergo a pre-season physical exam before participating in sports. It is important that this pre-participation physical evaluation (PPE) include a thorough medical history of the athlete, as well as family history.  These exams are best provided by the athlete's primary care physician, who is familiar with the student athlete.  The most updated PPE forms, which will help physicians know what history questions to ask, can be accessed here:

Appointments available for concerns noted on athletic physicals

Follow-up exam appointments are available now through the Children's Sports Medicine Center for school-age athletes whose pre-participation physical evaluation (PPE) showed a concern. Pediatric cardiology follow-up exams are available for concerns due to family history; heart murmur detected during a pre-participation physical exam; exercise-induced syncope; chest pain or palpitations; and other cardiac symptoms. Pediatric sports medicine follow-up evaluations also are available for other concerns.

To schedule an appointment or make a referral, call the Sports Medicine Center at 469-303-3000. Athletic trainers and physicians should complete the referral form and parents should bring the form to their appointment at Children's. The Sports Medicine Center is located on the Children's at Legacy campus, 7601 Preston Road (Preston at Hedgcoxe), Plano, 75024.
