- What health insurance do you have?
- What does your health insurance cover and what benefits are offered? (for example: medical, dental, vision, pharmacy, medical equipment)
- What physicians and hospital are in-network? Are these the physicians and hospitals you need to see?
- Call your insurance plan or visit the website to find a physician or hospital that is in-network.
- When does your health insurance end? Do you need to sign up or renew your plan annually?
- How much does your health insurance cost every month or every year?
- Are their monthly payments, deductibles, co-insurance, co-pays, and out of pocket expenses or maximums?
- If a new health insurance plan is needed, research and find the plan that best fits your needs.
