The team at our Plano – 15th Street clinic works to make your child’s primary care visits as smooth as possible. Whether your child needs a same-day sick visit or an annual well child check, we’re here to help your child feel better fast.
Appointments
Please call 469-303-8550 to make your child’s appointment. We’ll do our best to schedule it at a convenient time and see your child as quickly as possible. We offer same-day sick visits on Wednesdays from 9 a.m. to 12 p.m.

Our Vaccination Policy
At Children’s Health Primary Care Plano – 15th Street, our practice follows the vaccine schedule recommended by the Centers for Disease Control and Prevention and the American Academy of Pediatrics. We strongly believe in the necessity, benefit and effectiveness of vaccinations. On-time vaccination throughout childhood is essential because it helps provide immunity before children are exposed to serious, possibly deadly infectious diseases. Vaccines go through testing to ensure that they are safe and effective for children to receive at the recommended ages.

Medical Records Requests
Many records can be requested through MyChart patient portal or the Children’s Health mobile app at no charge.
If you are not able to request copies of your child’s medical records through MyChart or do not have a MyChart account, you may request medical records in person, by mail, or fax. A copy fee may be applied for this type of request.
