Pediatric Laboratory Ordering and Scheduling

How do I order a test?

1. Fill out the General Lab Requisition or the Metabolics Requisition or the MMP-7 Requisition
2. Place labels on requisition and all tubes.
   • Refer to our Lab Catalog for specimen collection, storage, and transportation requirements.
3. Package specimens accordingly
4. Packages should be sent via USPS, express mail, or courier to:

How do I schedule a collection?

Sweat Chloride collections are scheduled and performed by appointment only.

When calling to schedule, please have the following:

• Patient's Full Legal Name
• Patient's Date of Birth
• Patient and/or Parent Phone
• Ordering Provider's Name
• Office Phone
• Office Fax

A signed physician order must be faxed at the time of scheduling and/or delivered with the patient upon arrival.  Laboratory order forms are available by calling 888-730-DOCS and speaking with a Physician Referral Coordinator.
